Person 2 Person, LLC operates with the philosophy that the individual’s home provides the optimal environment in which most people thrive, develop, and mature as successful adults. Providing supportive in-home services not only benefits the individual, but also their families and their surrounding communities.

Overview of
"Supportive In-Home Services”
"Supportive In-home services" means the provision of community support services and other structured services to assist individuals to strengthen individual skills and provide environmental supports necessary to attain and sustain independent community residential living. Services include drop-in or friendly-visitor support and counseling to more intensive support, monitoring, training, in-home support, respite care, and family support services. Services are based on the needs of the individual and include training and assistance. These services normally do not involve overnight care by the Person 2 Person; however, due to the flexible nature of these services, overnight care may be provided on an occasional basis.
Supportive In-home services are usually supplemental to the care provided by a parent (natural, adoptive, and foster) stepparent, or similar caregiver. This service may also support an individual whose level of independence does not require a primary caregiver. A Supportive In-Home Services staff person will provide the training in the home or in the community. Supports are delivered on an individual basis according to the developed Plan of Care (POC) and are delivered with a 1:1 staff-to-individual ratio. Supportive In-Home Services are supports provided primarily in the individual’s home, which includes training, assistance, and specialized supervision to enable the individual to maintain or improve his health; assisting in performing individual care tasks; training in activities of daily living; training and use of community resources; providing life skills training; and adapting behavior to community and home-like environments.
Much emphasis will be on a person-centered approach that empowers and supports each person in developing his/her own lifestyle in the community according to his or her wishes and dreams.
Supportive in-home services do not include room and board or general supervision. This service will not be routinely provided for a continuous 24-hour period.
